Who Qualifies

Member Eligibility Criteria

The Loyalty AFC program is designed for individuals who need daily support and are living with a dedicated family member or close companion caregiver. To be eligible for the program, members must meet the following criteria:

  • Must be at least 16 years old
  • Must live in the same household as their primary caregiver
  • Must have MassHealth (Medicaid) or equivalent coverage
  • Requires assistance with daily activities such as bathing, dressing, or meal preparation
